Describe the reversal.
Before a change is released, write down what returning to the previous state would involve. A version rollback can be straightforward for one service and difficult for a change that modifies data. Keep those differences explicit instead of assuming that every deployment is reversible.
Assign the next action.
A useful plan identifies who can make the decision, where the runbook lives, and what signals would trigger a review. It can also name the dependencies that need a different recovery approach. Avoid putting secrets or production credentials in release notes.
Keep the plan close to the release.
The release record can point the owner back to the approved runbook. A readiness tool helps keep the question visible; it does not execute a recovery or replace the engineering systems that hold the actual procedures.
